- Underwold is an extremely tame horror film, nothing graphic is shown.
- A man is punched in the stomach and pushed back against a door, he is then headbutted in the chest and thrown on a bed where his head is held down.
- Pepperdine has her face slapped by Fluke.
- Roy Bain is attacked by a deformed person, his face is pressed against a stair bannister, Nicole smashes a beer bottle over the mutants head, the mutant and Roy trade puches in the stomach and head before Roy is slammed into a wall, Roy then grabs the mutant by the nuts, lifts him up and slams him into a lift safety door, the mutant punches Roy in the face and he tumbles down some stairs. The mutant approaches Nicole but Roy shoots it, no blood or bullet wound is seen.
- Dr. Savay is threatened with a needle held close to his throat.
- Roy is tied to a wall and threatened with a gun, Fluke injects him in the back of the neck with WhiteMare.
- Five mutants are shoot while trapped in a rundown warehouse, one is shot and falls out of a second storey window, another mutant is shot. These killings are not graphic and contain no shots of bullet wounds.
- Roy strangles someone with a piece of rope, he then grabs a piece of wood which is burning at one end and burns a mans nuts with it after sneaking up on him from behind and then knocks him out with the wood.
- A man is seen on fire who is also shot dead.
- A mutant stabs Fluke in the chest with a needle, the mutant is then repeatedly shot in the chest.
- Motherskille dies from gas poisoning.
- Dr. Savay pulls some loose skin of his cheek before his face and hand bursts into flame. His hand obscure the skin peeling from his cheek as the scene wasn't shot very well and very little is seen.
Profanity

- Profanity is used twice throughout the film:
- Once when someone says 'Gas the bastards' and once when someone says 'It's me you stupid bastard'.
Alcohol, Drugs & Smoking

- Character's are seen to drink and smoke throughout the film.
- Underworld has a central storyline involving a new addicitve drug, although it is mentioned and seen on numerous occasions it is never show being administered or the effects of being high on it.
