- Kitana's outfit is slightly revealing and skintight accentuating a bit of skin and showing a little amount of cleavage, though not as revealing as Nitara and Li Mei.
- Certain female fighters' outfits (Nitara, Li Mei) are revealing in nature, showing quite a bit of skin.
- Breasts jiggle when moving and fighting.

- As before, the violence is graphic in nature, showing quite a bit of bloodshed, including blood pooling on the ground beneath the fighters' feet.
- Fatalities still play a big part in the series, but compared to a few earlier titles some of them are not very graphic in approach. However, they are still graphic enough to warrant an M rating.

- Bo Rai' Cho is an alcoholic and uses the fighting style "Drunken Fist", which shows him staggering about in a drunken state. He has a jug of wine at his side everywhere he goes.
Frightening & Intense Scenes

- Some may still be shocked by the violent nature and Fatalities of the game.
- Nitara, as a vampire, has a rather creepy Fatality that involves her leaping on her defeated opponent and violently feeds off them as only a vampire would.
