Sex & Nudity

  • Occasional wallpapers of suggestively dressed women. (NO NUDITY)
  • An entirely optional and easily missed easter egg involves a character reacting to a photo of one of the above posters by stating that he will "save that for later"
  • Also, in order to distract an enemy, the player can choose to leave an inappropriate magazine in the enemy's path. It should be noted, however, that the player does not see anything explicit.

Violence & Gore

  • Blood can be disabled in all versions of the game for both cutscenes and gameplay. However, blood stains in the environment are not removed. Additionally, the player character will bleed a little when they are hurt too much in-game, but the bleeding can be healed.
  • The game is filled with gun violence and gore. As the game progresses, the player has access to more powerful weapons. Shooting an enemy in the head with a lethal weapon results in blood splattering (same thing happens if you shoot them in the stomach or chest).
  • Many different scenes where there's dead bodies lying around and blood spattered everywhere (there's a reason why this game was given an 'M' rating in America, and a "15" in the UK).

Profanity

  • Mild language.".

Alcohol, Drugs & Smoking

  • We see a character start to smoke a cigarette. The player also obtains cigarettes and has the choice to smoke them. Smoking them results in the Life Bar decreasing slowly, but stops trembling of the hand when sniping, allowing the player to kill things more easily.
  • One of the characters makes references to being a heavy drinker in the past.

Frightening & Intense Scenes

  • There are tons more scenes in the game that would never be suitable for kids.

Sex & Nudity

  • At a later point in the game, Raiden rescues President Johnson from his holding cell, and Johnson grabs Raiden's crotch, shocking Raiden, causing Johnson to be surprised that Raiden's a male due to his androgynous appearance.
  • Also, very late into the game, Raiden is stripped of everything, including his clothing, after being "Betrayed" and "Captured" by Mr. X/Cyborg Ninja/Olga Gurlukovich, and spends most of the level sneaking around while naked (covering his genitals with his hands) until he regroups with Snake and recovers his clothes. During the same sequence, if a guard walks in on Raiden being restrained, he'll make a comment that implies that he envies Raiden slightly regarding his genitalia size.
  • After the Harrier battle, one of the guards, while on sentry duty, decides to take care of business and pees off the side of the bridge.

Violence & Gore

  • The main villain, Solidus Snake, strangles Olga Gurlukovitch and then shoots and kills her in the head with a P90 after learning of her betrayal.
  • In one scene when B1 Shell Core 2 is flooded, Raiden finds Peter Stillmanscorpse with blood around it.
  • Raiden slashes Solidus Snakes spine gallons of blood spurt out very disturbing and gory.

Frightening & Intense Scenes

  • Emma Emmerich's death can be depressing. Otacon, who's Emma's brother, can frequently be seen mourning after this happens.
  • It may also be upsetting how Olga must die in order to ensure the safety of her child towards the end of the game.
  • The Patriots can be disturbing for their overwhelming lack of human empathy, and their revelation breaks the 4th wall, which may scare some players not acquainted with series creator Hideo Kojima's humor.
  • The scene where The Patriots explain their motives turns into an analysis of the human mind that some may find offensive or upsetting, as it indirectly insults the player's intellect.
