- In a room during the Brookhaven Hospital level, there's a poster with a woman in a bikini standing in a suggestive manner.
- A lot of sexual implications.
- Pyramid Head's first appearance sees it murdering two monsters by shoving them into a sink. Although this scene can be confused as appearing to depict rape due to the position of Pyramid Head and the monsters, the game designers have since confirmed that this is not the case. Despite this, it is made to appear as a sexual assault to align to the games themes.
- The Abstract Daddy is a boss that's all about sexual abuse also the mouth resemble a vaginal orifice, also symbolising sexual abuse or rape the abstract daddy also screams when you attack it what also is symbolizing helplessness from the sex victim. The Abstract Daddy is made to look like a bed with sexual abuse happening on top of it it's deeply disturbing.
- Flesh Lips is another monster in Silent Hill 2 the monster what's indicating vaginal imagery from James wife Mary.
- Mandarin is a Another monster that symbolizes vaginal imagery, which touches more on his sexual frustration.
- The Mannequin is a manifestation of James natural urges and inclinations, as frustrated by his wifes ailment. Accordingly, Pyramid Heads introductory cutscene has him sexually abusing two Mannequins. The Mannequin may also be an image of innocence and helplessness being misused or otherwise destroyed by an oppressive, masculine forcea symbol that is later touched on in Toluca Prison, where James finds the Tablet of the Oppressor.

- This game contains aberrant themes that some may find upsetting or disturbing (Incest, Sexual Assault) However, most of the sexual content is implied and symbolized rather than explicitly shown.

- Some language can be heard in the game. There are 2 uses of "disgusting piece of sh*t" in one scene along with a use of "a**" and there are several uses of "d*mn" and "h*ll", especially towards the end of the game.
- Occasional uses of "hell" and "damn". There's 2 uses of "shit" as well.
- Name calling (fat disgusting pig, fart-face, ugly, stupid.) For the very easily triggered, I guess.

- The Heaven's Night club that James and Maria visit has a cocktail bar with bottles and glasses of wine and beer on it.
- James visits a bar at one point near the end of the game.
- In one of the bathrooms there is a powder on one of the floor.

- This is one of the most disturbing and scariest video games ever.
- If silent hill 2 was a movie, it would be r rated for bloody violence, and sexual content, and scary scenes
- Silent Hill 2 is widely known as one of the most disturbing and unsettling video games of this generation. There are many themes of torture and abuse, including monsters that represent very disturbing themes of violence, including rape and sexual abuse (Abstract Daddy, Flesh Lips, etc). The atmosphere is very unsettling at most times, especially during the opening sequence when you walk through the forest.
- The game uses a disturbing and oppressive atmosphere, as opposed to "jump scares" to genuinely disturb and unsettle the player, rather than making them jump.
- Several environments are disgusting, viscerally intense and horrific.
- There are also emotionally depressing moments as well (RAPE THEME)

Violence & Gore
- A character goes insane and attacks you with a pistol.
- A character walks up the stairs of a burning staircase and commits suicide.
- The protagonist and his companion flee a monster and escape into a lift. However, the companion doesn't reach it in time and it's implied she's stabbed with a large knife and murdered.
- James reaches a jail cell and finds a lady dead, her body and the jail bed covered in blood.
- A woman, tied up, has a spear thrust into her back (the wound is not seen onscreen) and is killed in front of James.
