Sex & Nudity

  • One boss fight includes a giant spider with a human torso. Her bare breasts are seen throughout the fight.

Violence & Gore

  • One scene shows a lady's head getting ripped off, along with her spine.
  • This is WAY more violent then the previous installments and due to much better graphics, it's also much more graphic.
  • Blood can often be seen splattered on the walls and floors, and oftentimes threatening messages are written in blood.
  • While demons simply disintegrate into ash upon death, "human" enemies such as zombies will often explode into a gory mess when shot at, allowing blood, bones, and sometimes brain matter to be visible.

Profanity

  • Several uses of mild profanity, such as "Hell", "Damn", "Ass", "Piss", and "Shit". Despite this game having the most profanity in the series, it's not that bad because they aren't heard often and are barely heard.

Alcohol, Drugs & Smoking

  • Characters may be seen smoking.

Frightening & Intense Scenes

  • Lots of human disfigurement, which looks really scary.
  • The game has an incredibly dark, creepy atmosphere throughout.
  • During the course of the game, you fight hordes of terrifying monsters including zombies and demons, the latter of which come in a variety of forms and are often horrifying in appearance.
  • At one point in the game, the protagonist hallucinates that a woman is calling for him, claiming that demons stole her baby. If he follows the voice, he eventually comes to a dead-end where the screen turns red and loud baby cries echo through the hallway. This can be very creepy.
