Sex & Nudity

  • Vamp is shirtless and make sexual movements majority of the time. During a cutscene Naomi touches his body and reaches for his penis (where he puts his knife) but he moves her hand away.
  • In the beginning Snake can find a statue of naked Roman men. If he hides next to the statue he will get into a pose and use on of his hands to cover then penis on the statue. After a couple of second the penis on the statue will break. This is to be taken as a joke, which Metal gear Solid 4 has a lot of in-game humor despite the dark story.
  • Snake can find real pictures of Japanese female stars on wall, lockers, and other places. They are all young and sexy looking.
  • Snake tells Raiden that his girlfriend is married to an older man. Raiden then tells Snake that "she means nothing to me now," and that Raiden's new place is in the battlefield.
  • Several women (and some men) are wearing very tight costumes that can be seen throughout the game, this includes the protagonist who's seen in a very tight suit. This includes detailed outlining of the buttocks and breasts.
  • Johnny is seen defecating inside a barrel, and his buttocks are exposed when someone exposes him.
  • A woman keeps getting very close to a man's face in a flirtatious manner during a conversation, she later pulls him into a closed compartment and sex is implied.
  • A man and a woman kiss passionately while on the ground, he's later seen hitting her butt jokingly; the same man tries to grab another woman's buttocks in a sexual, but comic, fashion before he's stopped.
  • Several women sport some very heavy cleavage, including a middle-aged woman who has her shirt open to reveal some quite deep cleavage.
  • All B&B bosses use sexual movements as attacks

Violence & Gore

  • Many soldiers hallucinate and go crazy due to their implanted chips malfunctioning. They scream, attack others, and act like zombies.
  • A long and violent fight between a human cyborg ninja and an undead human vampire. Lots of stabbings and blood, but no gore.
  • Snake gets his face burned. At first he hides it behind a bandage but he exposes it to Raiden, telling Raiden that he still has a life to live while Snake only death waiting for him.
  • A violent and epic five minute fight scene towards the conclusion of the game.
  • A limbless human ninja cyborg battles against soldiers. He summons electricity to fry the soldiers. He then hold his sword in his mouth, ready to attack.
  • An old man is known as a womanizer.
  • A man gets tortured by another man. It is rather quick. The man stabs him with a knife and electrifies him.
  • A man orders his soldiers to destroy an entire army after their weapons fails to work. It is a long and intense scene ending with a living corpse being burned alive. A women attempts to save the corpse only to be burned herself. She dies later next to another man who is also burnt.
  • A man has a metal arm.
  • A man blames himself for giving away his team's position to the enemy and begins to freak out, smashing his head against the wall. The leader of the team slaps him and tells him to stop being a "dumbass."
  • Snake dislocates his shoulder after fighting Metal Gear Ray.
  • Vamp's death is rather gruesome. He squirts a lot of white blood.
  • A character commits suicide by injecting a needle into the neck. Another character watches and cries.
  • Blood can be disabled for gameplay and most cutscenes in the options menu.
  • Weapon violence throughout. The game takes place during a war so people are seen getting shot and shooting at each other. Whenever someone is shot, they usually spray a thin mist of blood, several cut scenes have lots of battles; in a scene a group of people are shot, we see blood holes open up in their body as they're hit with bullets. In another scene, a man is shot in the head, we see the bloody hole between his eyes. In the beginning of the game, a truck driver transporting various militiamen ends up being sniped in the head by a PMC sniper, with blood spraying on the windshield. This also causes the truck to crash into another truck, sending the troops flying at a velocity that's implied to have killed or severely maimed the soldiers.
  • A group of women wear Hi-Tech devices that resemble certain animals; the vehicle suits resemble an octopus, a raven, a mantis and a wolf. These characters have very disturbing background story and are actually quite terrifying. The octopus uses her metal tentacles to impale a couple of people in a cutscene, opening visible holes in their bodies, in another scene she wraps her tentacle around a man, covering him completely, before she squeezes, we hear loud cracking noises and see blood spray from between the tentacles, the man is seen dropping to floor with an implied broken body. The wolf one actually attacks and bites several men in one scene, she also uses her whip to beat them with bloody results, it's not gory but it's quite disturbing.
  • A Gekko is a tank with long muscular legs instead of wheels, the legs are actually made out of artificial stemmed muscular tissue. There're several of them throughout the game. In one scene, many jump around in a battle, killing soldiers whom upon they land, the camera spans away but blood sprays into sight. In the same scene they're ramming into soldiers and kicking them too, it's implied that the victims died during the battle; in another scene they attack a market filled with civilians, no graphic deaths are shown.
  • Several sword and knife fights, blood can spray and usually pools underneath the victims. Many fights are choreographed and simply prolonged. In a scene, a man cuts of the legs of three Gekkos with his sword, the legs bleed heavily.
  • A man's arm is trapped under a rock, he breaks it at the shoulder (we hear a loud thud and he screams in pain), he then uses his sword to cut his arm off, this isn't shown on screen; we, however, see an immediate shot of him from above where he clearly is missing the limb and white blood is spraying out of the location.

Profanity

  • S***, B***h, and other moderate words are used infrequently.
  • "F***ing" is used once in a non-sexual fashion.

Alcohol, Drugs & Smoking

  • Not related to a particular drug but a man and his monkey is shown to be addicted to drinking some sort of soda.
  • Constant smoking from the protagonist; his smoking forms a sub-plot. It's also implied that one of the character's pet got addicted to cigarettes due to it snatching it from the protagonist.
  • Experimental drugs are also a part of the plot.
  • It's implied someone's drunk.

Frightening & Intense Scenes

  • A lot of talk about government control, future wars, PTSD, and meaning of life.
  • Some talk about child soldiers. Two of the characters were child soldiers, Raiden and Dreblin. Dreblin has a scar on his head. He hints to Snake that he got it from his days as a child soldier.
  • Snake is extremely suicidal in the game. He often talks about dying.
  • Like all the Metal Gear Solid games, the story is intense and features frightening, disturbing and violent characters.
  • Some of the shootouts are intense.
  • As well as the B&B boss cutscenes.
  • The backstories of the B&B members may be disturbing to some. Namely:

Frightening & Intense Scenes

  • Final cut scene involving Big Boss and Snake may be emotional to some.
  • The entire ending is very bittersweet for all involved and is very, very depressing. With Snake's life ending soon because of FOXDIE, Big Boss and Zero dead, and everyone else, the ending is depressing but is not all dark.
  • Laughing Octopus: Her entire Scandinavian town was executed by an anonymous cult, and she was forced by the same cult to brutally massacre her family and friends, as well as to enjoy having to kill them by laughing.
  • Raging Raven: She was placed in a cage with other children by soldiers (presumably from Indonesia), and was regularly beaten to a pulp by the soldiers in their fury, and was later left to die. She was later left with the kids to become snacks for Ravens, but before they could feast on her, she tore them apart, and later tracked the soldiers down and at nightfall started killing the soldiers, even going as far as to kill the civilians that they captured, all the while cawing and screaming in an uncontrollable rage.
  • Crying Wolf: Her family and friends were slaughtered by a faction in Africa due to ethnic cleansing, and left her and her baby brother the sole survivors, as well as refugees. she later accidentally smothers her baby brother to death when trying to keep him from crying and alerting soldiers to their presence. Afterwards, feeling immense guilt, she later hallucinates a wolf following beside her, and after getting to a government run camp, she starts being disturbed by the cries of children, and later starts believing that a wolf is killing the children (in reality, she was the one killing them).
  • Screaming Mantis: Probably the most disturbing backstory. After her home village was burned to the ground in a war, she ended up being separated from her family, and was being pursued by Death Squads in South America. She tried hiding inside the basement of a building, but soon discovers that the building was actually a homemade torture chamber, complete with corpses littered around the basement. She couldn't just leave due to the military presence of the building, and she later can't even leave the basement due to it being locked up. Because of this, she ends up being traumatized by the screams of tortured villagers day and night. She later began to hallucinate after drinking contaminated water, hallucinating of a Black Praying Mantis that taught her how to block her ears and then eat the male corpses, essentially commiting cannibalism to survive the weeks locked up in the building.
  • Snake, during the ending, attempts to commit suicide by placing a gun in his mouth and firing it, but is later revealed to have evaded the option at the last second. The idea of suicide may be distressing to some players.
  • Major Zero and Big Boss's deaths maybe upsetting. Big Boss kills Zero by cutting off his life support, and then dies due to the effect of FOXDIE within Snake.
