- A character has to save another character by ripping an electronic device on the center of her chest.
- One of the main character's unlockable costume is a tribal costume, consisting of a loin cloth and a bra.

- Regular enemy types can be decapitated using firearms. A gory sound is heard, the head explodes into chunks and a parasite appears from the neck stump. Rather graphic.
- Shooting the enemies in the body causes blood splatter to appear.
- Explosives are capable of disintegrating enemies into a splash of blood. Rather graphic.
- Two enemy types in the game wield giant melee weapons and are capable of impaling the player through the chest with them. Graphic.
- Disfigured bodies covered in blood can be found within the game environment.

- Players will hear strong language including "sh-t," "Damn," "hell" and "bitch." The f-word is used once or twice during cutscenes, but they may be skipped.

- A scene where a man is forced to ingest a parasite. He has a seizure then mutates into a zombie.
- The game contains several jumpscares.
- Frightening scenes of humans mutating into monsters.

- A character touches Wesker suggestively during a scene. She rubs her hand over his stomach and shoulders.
Violence & Gore
- Towards the end of the game, player encounters a monster type named "Reaper", which has a special attack where the monster holds the player and repeatedly stabs him in the chest as large amounts of blood come out. Rather graphic.
Frightening & Intense Scenes
- The scene where Wesker is about to be sucked out of the back of a flying airplane grabs Sheva's leg to take her with him while she is holding on not to be sucked out of the plane. Chris reacts quickly once she lets go and saves her. The scene is very intense for some viewers.
