When the steel mill foreman calls Jack and talks about his coworker who becomes the Hulk,
Why didn't the foreman tell Jack what his name was as opposed to a generic description?
He could have said, "His name is David "Brown" (last name being an alias)
Lou Ferrigno's foot-protecting green slippers can be clearly seen when the Hulk escapes the factory.
Transformation in the steel mill, the fallen hardhat can be seen being taken out of the scene.
During this story, which takes place largely at the "National Register" editorial offices in Chicago, Illinois, Jack McGee books a commercial flight from Chicago to Gary, Indiana, when he gets a report of a Hulk sighting there. However, in real life Gary, IN, is less than 40 miles from the southeast Chicago city limits.
NOTE: Not an error booking a flight from Chicago to Gary. Chicago is big and stretches far. Flights exist from O'Hare to Gary, for example. There are many businesses in the northwest corner of Chicago, in fact it is more likely a business would be there instead of right close to Gary. Such short flights are known as "puddle hoppers".

Usually David doesn't stay long in one place after the Hulk has been seen, specifically so that Jack McGee can't track him. However, when the Hulk was first spotted running through the steel mill, David inexplicably continued to stick around.
