It's not possible someone who knew what ether smelled like, as a surgeon would, could mistake water for it. Ether has a very low vapor pressure. As soon as its container is opened, the fumes are obvious to anyone close by. Hawkeye and Trapper would have noticed as he was pouring that something was wrong due to the absence of its very strong smell.

Maj. Houlihan transferred Nurse Cutler out of the 4077th, but only a unit's commanding officer can transfer personnel (even LTC Blake professes that he can't do anything about Houlihan's action). Furthermore, such a transfer would still have to be approved by either regimental headquarters, division headquarters, or I Corps. Additionally, such a process would take quite a bit of time, not be effective almost immediately, as portrayed.
