At the beginning Mannix is shown traveling by helicopter in a shot taken from outside the vehicle. The pilot is wearing glasses and has a set of headphones over his ears, with a microphone in front of his mouth. Then the shot switches to inside the helicopter and the pilot suddenly has no glasses, and has the headphones on the outside of his hat, not over his ears.
Barbara Montfort scratches her arm deeply enough to draw blood in an attempt to frame Mannix. In subsequent scenes there are no scratches on her arm.

When the mysterious figure removes brake fluid from Joe's master cylinder the engine bay is red. The car has been repainted since Chrysler Corporation cars were always the same color under the hood as the body, and all of Joe's cars were all green from season 2 to season 5.

When Mannix is examining the Montford apartment he notices four dimples in the carper indicating a piece of furniture had been moved. He asks if they had moved something that was 6-7 feet wide. Such a large piece of furniture would not have had their feet so close together, indicating what was there must have been a much smaller cabinet than that.
Barbara Montfort scratches her upper arm until it bleeds; the cops burst right in when she yells for help. They arrest Mannix. Not once did he say he didn't do it, nor did he show them his clean hands, nor did he tell the cops to check her fingernails. They simply arrest him without proof whatsoever, and he submits to it.
