At 41:18, when Major Zelinko reviews the session log of the glass cage, it did not have a check-out time for session #27. However, at 41:53, when Zelinko shows the log to Capt. Gulka, the check-out time is clearly visible.
When Cinnamon releases the towel-wrapped package from the secret briefcase compartment onto the bed in the jail cell, the package is flat and rectangular and fitted nicely in the secret compartment. When Willy and Barney return to their cell, the package is big and lumpy and could not have fit into the secret briefcase compartment.
The brief case that Peter Graves' character is switched with Barbara Bain's character (to hold the tools for Greg Morris' and Peter Lupus' characters to break out of the prison cell) is clearly different as seen from the bottom of the cases. When Bain's character inspects the cell, she flips a switch that opens a "door" in the bottom of the case to slip out the tools wrapped in a towel. Earlier shots of the same brief case shows no such door panel or switch existing.
When Reisner is first being tortured, he starts to flinch when they turn the sound up from -40 decibels to 0 decibels. 0 decibels is the typical minimum sound that the ear can hear.
When Bochner is viewing Phelps and Hand at the entrance gate, he flips the directional control from "vertical" to "horizontal" and the camera moves without any other control being touched, and also stops without any other control being touched. It also moves in the desired horizontal direction (right) without any control specifying whether the horizontal motion is supposed to be left or right.
As Barney and Willie wend their way from their cell to the master control room, it seems implausible that an ultra maximum security prison would not have a presence of guards along their route. When Willie pulls the switch to kill the lights, guards then come running from all directions.
They don't think they need guards there because the floor is electrified. When Willie pulls the switch, the floor is turned off so the guards can cross the floor.

When Barney and Willy are making their way to the control room they use a flare gun to blind the surveillance camera. When they show the camera there is a large rectangular spot on the wall beside it. When Barney fires the "flare" this spot starts spewing sparks showing where the flare was hidden.
At 47:38, as the team escapes in their vehicle, they go through a gate marked "Zona Restrik". But behind that sign is a sign saying "Welcome to Willow Grove". It is a reused scene from "The Carriers".
