"A Pain in the Neck" finds Fred about to receive the Watts Businessman's Man of the Year Award, but unable to attend the ceremony after throwing out his back along with a few other things. Aunt Esther's presence only increases his agony: "you just as stubborn as an old jackass" "and you just as pretty as one!" Nancy Kulp makes the last of her five appearances as Officer Hoppy's mother May, bringing in Panjib (Tim Herbert) to perform 'transcendental consciousness' to aid Fred's ailing back. Sitting on the floor with 'Left Jab' doesn't agree with Fred: "are you having trouble with your flower of tranquility?" "no I'm havin' trouble with my Fruit of the Loom!" A deserving testimonial is delivered by Whitman Mayo's Grady Wilson, who only appears on the show twice during its next and final season: "when you think of Fred Sanford you think of junk!" Saving the best for last, Fred reveals his old nickname from his youth in St. Louis, 'Rudolph Vaselino!' (a nod to Stan Laurel).
