Sex & Nudity

  • The blond female A.I. has quite revealing tops as you can see the sides of their bare breasts.

Violence & Gore

  • A semi-rare animation may occur if you shoot a bullet weapon or stab your bayonet in the neck region of a generic soldier. This death animation involves the soldier holding his bleeding neck while gurgling and choking loudly. A huge fountain of blood also spurts, even though the enemy's hand is blocking the wound.
  • Frequent, frantic, intense, graphic, and realistic violence. When you shoot someone, large blood splatter comes out. You can blow off heads and limbs with guns, axes, and grenades, which results in a waterspout of blood coming out. You can also get upgrades that increases damage for your weapons and when you shoot, one shot with the upgrade instantly blows up someone into pieces.
  • When you get hurt, blood splatter appears onscreen. The more you get injured, the more blood appears.
  • Later in the game, you can get sci-fi weapons and power upgrades that can either vaporize people or shock them with electricity. Sometimes, you can briefly see their bones before they die.
  • The shield power, when fully upgraded, allows you to disintegrate anyone who touches your shield. The only thing that remains are bones.
  • You can get a flamethrower or an explosive barrel and burn people with it, resulting in burnt corpses. You can also burn them even more, resulting in a more burnt corpse.
  • The mythical creatures have much heavier gore as they explode into a mist of goo-like blood, disintegrate into fleshy pieces when hit by certain weapons, and can be dismembered like regular humans.
  • Some punching, melee attacks, and hitting (some result into blood splatter).
  • You can optionally upgrade your rifle with a bayonet and stab people with it or upgrade the tesla gun (a firearm that fires arcs of electricity) to make it shock people when you melee with it. Very disturbing.
  • The game features some cinematics with people being executed or shot in the head.

Profanity

  • Some uses of "shit" along with milder profanity (e.g. "damn," "hell") and some name calling, such as "swine."
  • Occasionally, the Nazis will curse in their own language.

Alcohol, Drugs & Smoking

  • Several Nazis and civilians can be seen smoking while not in a combat situation.
  • Later in the game, the player can go into a bar full of people drinking and holding mildly alcoholic drinks.

Frightening & Intense Scenes

  • The violence and gore can disturb some players.
  • The Veil creatures' appearance can be frightening due to their violent and cruel nature.
  • The cutscenes are quite disturbing and frightening as the player may not know what is happening
  • Some Nazis are seen executing or beating up civilians.
  • The stealthy nature of the Veil Assassin and the "Sniffers" can be very suspenseful as they tend to come out of nowhere and make horrific laughs or sounds.

Violence & Gore

  • In the end, we see B.J. Blazkowicz punch and throw a German guard off a zeppelin.
  • A cutscene shows Nazis get vamporized by a Veil generator as only one officer transforms into a Despoiled with some bits of flesh falling off.

Frightening & Intense Scenes

  • The 5th main level, Hospital, is the possibly the most disturbing level in the game. Introducing an scary and stealthy enemy type, a new gruesome boss enemy and a tesla weapon. The cutscenes are mostly filled with bloody violence and horror themes.
