Sex & Nudity

  • Zoey sometimes makes references about herself being the only woman left on earth (implying everyone would need to have sex with her to repopulate the earth).
  • Francis sometimes makes sexual comments about Zoey, and himself.
  • While in the hospital, the zombies have hospital robes on, and their bare bottoms are visible.
  • The Witch does not wear many clothes. She is basically a scary-looking young adult female wearing short shorts and a bikini.

Violence & Gore

  • Intense, non-stop, and very bloody violence.
  • Arms, legs, will come off and heads will explode in very bloody fashion.
  • Everytime you kill a zombie by shooting him, blood sprays and splatters on the walls.
  • You can blow up and dismember zombies legs, arms and even heads in a gory way.
  • You can throw a pipe bomb, which attracts the attention of most zombies by getting them around it, and when it explodes, a bloody cloud exparses and all arms and legs are blown up.
  • The first "Special Infected" is the Boomer, which is a fat zombie that vomits on players and makes zombie hordes attracted to you. If you shoot him, it explodes making a bloody mess and his half of its blown dead body exposes brief organs and intestines.
  • The second Special Infected is the Hunter, which screams loudly and jump towards you. If the Hunter pounces on a player, he begins clawing away until that player is rescued by another player or bot.
  • The third Special Infected is the Smoker, which has a long tongue and uses rooftops to "snipe" players with his tongue. If he connects, the player is dragged towards him and strangled until another player or bot rescues them, or the Smoker is killed. Smoker deaths result in a large cloud of smoke that obscures vision and makes survivors cough.
  • The fourth Special Infected, which cannot be controlled in versus mode, is the Witch. When nearby a Witch, you can hear her crying, along with creepy music. If you startle her (by getting too close, shining your light on her, or shooting her), she runs after the perpetrator and pins them to the ground while slashing at them. Again, the pinned player must be rescued by their teammates.
  • The final Special Infected "boss" is the Tank, a very large Hulk-like zombie that can throw vehicles and rocks around, knocking players to the ground. After being killed, many bullet wounds can be seen on the Tank's muscled body.
  • When any player dies, their dead body may leave behind a large pool of blood, depending on how they died and what their status was before death. On the game display, a skull icon or X appears next to the player's name.
  • When you fall from a great enough height, you can hear a crunch sound, implying a broken bone. This doesn't directly affect walking speed.
  • If you kill zombies/infected very nearby, the screen display becomes splattered with blood briefly.

Profanity

  • "Ass" is used infrequently.
  • Excessive usage of "Shit" in dialogue, alongside other mild-moderate level words ("Hell", "Goddamn", "Bastard", "Effing"). As with other Valve titles, "Fuck" is deliberately not said in the dialogue.
  • One of the characters, Bill, is a Vietnam War veteran and uses the term "fubar" from the movie Saving Private Ryan in one line. ("I am seriously Fubar!", when very low on health and having been revived twice already.) It's probably the same meaning as the f-word.

Alcohol, Drugs & Smoking

  • Throughout the whole game, you see the character "Bill" smoking a cigarette.
  • Characters can store and use pain pills to gain temporary health.

Frightening & Intense Scenes

  • The survival aspect and the appearance of some of the "boss" zombies may frighten players.
  • The zombies in general may be frightening.
