Sex & Nudity

  • A couple visual, audio, and text-based references to sex ranging from posters and mannequins with half-naked women (no nudity.)
  • Explicit graffiti (e.g. "cheerleader whore" scribbled on the bathroom wall, near a cartoon drawing of a topless woman)
  • To sexual questions the player must answer in a psychiatric exam ("I enjoy role-playing during sex," etc.)

Violence & Gore

  • As with past Silent Hill video games, this sequel involves demons that chase you -- but unlike its predecessors.
  • Harry doesn't fight back as he has no weapons.
  • He just runs, but is quite terrifying.
  • The violence is seen if the creatures attack you (opposed to the other way around).
  • Blood can also be seen smeared on walls and floors.

Profanity

  • Some inappropriate words can be heard including "s--t," "assh-le," "bitch," "damn" and "hell."

Alcohol, Drugs & Smoking

  • During the psychological testing phases, Dr. Kaufmann will ask you questions and discuss topics about drugs and alcohol. In addition, the questions you answer will influence the course of the story and Harry's behaviour.
  • You will notice bottles of alcohol in Dr. Kaufmann's office.
  • At the start of the game, a man pours liquor into a glass and sips it.
  • One flashback sequence shows a girl smoking a cigarette.
  • Some drug references on posters including references and imagery related to cocaine and marijuana.

Frightening & Intense Scenes

  • The entire game has strongly unsettling undertones, with dialogue and themes meant to touch upon real-world fears such as loneliness, alienation, guilt, uncertainty, fear of death, loss of loved ones, fear of sexuality, and fear of oneself.
  • The game world is stark, silent and eerie, and the intense "Nightmare" sequences feature grotesque and disturbing monsters that relentlessly chase the player and shriek when they are close.
  • While the game is not explicitly violent, it is intense and at times highly disturbing - definitely not recommended for younger players.

Violence & Gore

  • On returning to the flat, Harry discovers Lisa, who convulses for half a second and then starts bleeding from her eyes and nose in large quantities.
