Natalie mentions to Monk that before Richard Meckler, she had never seen anyone have a heart attack. But in Mr. Monk vs. the Cobra (2005), she was not only present as Chris Downey had a heart attack, but was the first to identify it as such.
When Pressman is shown in his garage trying to put on his tool belt, the tool belt buckle is in the front, between the two pockets. During the flashback at Monk's party, Pressman is shown trying to buckle the buckle, but this is now being done behind him.
Dr. Z says that sodium fluoroacetate is so "fast-acting" that it can kill within 10 seconds. In reality, the compound can cause symptoms within 30-60 minutes of ingestion.
Trash compactors generally do not have shredders. Shredders are designed for one type of material (paper, plastic, wood, cardboard, etc.). They cannot handle a variety of regular trash - or the tire iron that clogged the machine. Even if this compactor was meant for just paper, there was no signage indicating this.
The victim screamed longer than he would have having gone through a shredder with all the blades that are shown.
The first victim was a janitor but the building manager called him the maintenance man. "Janitor" and "maintenance man" are not interchangeable terms and the building manager would know this. Janitors clean while maintenance fixes things (and get paid more). Also, the janitor was shown going a long distance to throw out half a bag of trash. A real janitor would take out all the trash at once instead of making multiple trips. It is also very strange that the janitor was throwing the garbage into the shredder/compactor by handfuls rather than dumping it by bagful.
"Maintenance man" has been used by many people as a term for "janitor" since the 1970s. It is not accurate - the two are not the same - but it has been used that way.

While moving the dumpster, one of the men says to dump it in the river. There is no river in San Francisco. It would have been a simple fix for the actor to say "dump it in the bay."
When they are at the janitor's place and Monk picks up two handfuls of shredded paper and then puts them back where they came from, he should have asked for a "wipe" but he did not. Quite not the way Monk would act after using his hands that way.
