Sex & Nudity

  • There is a character called from the game "Mighty Jill Off", a reference to the sexual action "jack off".
  • One character is called a "pervert" and licks shoes.
  • Some characters are naked, but no genitals or nipples are seen, such as Meat Boy and Bandage Girl.
  • One character is dressed in a BDSM outfit. However this is not explicit and the reference will likely go over younger players' heads.

Violence & Gore

  • One enemy later on in the game has guts spilling out of it.
  • Live animals such as squirrels, rabbits and birds can be found in certain levels. Upon contact with saws, these animals die with a quiet squeal and a small explosion of blood.
  • Meat Boy leaves blood everywhere he goes, which serves as a helpful reminder of where you've been.
  • Some characters explode when killed, and blood is seen.
  • Comic physical violence throughout, with characters looking lumpy and deformed after being beat up.
  • Dr. Fetus creates a living doppelganger of Meat Boy out of his feces, which the game refers to as "Brownie".

Profanity

  • Use of the words "pervert" and "Hell".
  • The pause menu shows an infinitely repeated animation of Dr. Fetus giving the finger.
  • Many uses of the middle finger by Dr. Fetus in almost every cutscene he's in.

  • This game is known for its unrelenting punishingly brutal difficulty. As such, it is not for the easily frustrated.

Sex & Nudity

  • In the Dark Ending, a fetus can be seen inside of Bandage Girl, implying that she and Meat Boy had sex.

Violence & Gore

  • A squirrel is bloodily decapitated by a circular saw. This happens fast and is not particularly graphic.
  • Meat Boy kicks a small blood monster, resulting in it looking deformed. Meat Boy then apparently ends its suffering by suffocating it to death with a pillow off-screen. However, the blood monster is shown to be alive during the game's end credits.
  • In the Dark Ending, Dr Fetus is repeatedly stamped on and dies in a small explosion of blood.
  • Meat Boy has unlimited lives, but whenever he dies his corpse is sent to Hell. While this is not directly shown, it is hinted several times throughout the game.
  • After the fight with "Big Slugger", many squirrels are killed. Their dead bodies and decapitated heads are shown in a cutscene. No blood, but this may upset some players.
  • Meat boy tries to escape a hospital filling up with blood, while avoiding a blood monster.
  • Meat Boy is supposedly killed by Dr. Fetus and ends up in Hell. (This isn't explicitly shown, but is implied.) While there, he sees a pile of dead Meat Boys, which he realizes are his past lives.
  • Dr. Fetus causes a nuclear apocalypse, supposedly killing everyone except, conveniently, himself and the main characters. (This is also not shown, though a squirrel is seen becoming a skeleton from the blast.)

Frightening & Intense Scenes

  • The boss "Little Horn" is a giant Meat Boy made out of smaller, dead Meat Boys. This character is very large and its fight has lots of Meat Boy corpses falling from the ceiling, fire and shake effects.
