- Throughout the game, your character kills "screamers" by hitting them with numerous weapons. The most violent of these is a fire axe which can be dug into the screamers neck, squirting blood out while a crunching is heard.
- Numerous instances of bizarre imagery including bodies that explode open.
- Murphy (your character) becomes bloodied and weak when he's injured.
- Many attacks by other creatures, not all are bloody or gory.

- It's not often, but the game has some strong profanity, such as "f--k" and "s--t," as well as other potentially offensive words like "ass," "damn," and "hell."
