- At a cemetery, there's a funeral for Theo. The priest says the eulogy while the onlookers watch. Patrick tells Gino that he couldn't get anything from the police. Sam collapses and sees Big Daddy on the outskirts of the crowd.
Later, Sam wakes up in the hospital and the nurse stops him from pulling out his IV. Sam recognizes the nurse, who tells Sam that he never asked what he did for a living, but it was hard to hear anything under the leather hood. Theo comes and tells Sam that he prefers to be called "Doctor", and tells Sam that he has a severe fungal infection and it doesn't seem to be responding to treatment. He tells Sam that he's in the only hospital that would take him, and people are afraid of the spreading infection.
Theo leaves Sam's room with Sam hooked up to an IV. Sam wonders if he killed Theo, and Theo asks him if he intended to. Theo asks about the other lives that Sam "devoured", and he spent his life avoiding the consequences of his life but they caught up to him. He tells Sam that he said was coming caught up to him, and leads him to a room. Sam gags on the stench, and Theo tells him that no one else will come in. Theo sits on the bed and says that the patient Danny was admitted a month ago with the same infection that Sam has. He reminds Sam that Danny one was the first person they opened their relationship up to. Theo says that Danny used to be a talented writer, and Sam admits that he never went to see it. He explains that Danny thought Sam hated his play, and that's the impression left.
Danny starts evacuating his bowels, and Sam calls for help. Theo tells him that no one is coming, and Sam shows his clients things that they've never experience, just like he's doing with Sam. Sam insists that it's not his fault, and tells him that there's more to see.
Theo leads Sam to a patient in another room. The patient is Stewart, and Theo says that Stewart was sick before he met Sam. Sam tells Theo that he was coked up and always pushed the limits, even when he put Stewart in a cage. Theo tells Sam to kiss Stewart. When Sam says that it isn't his place, Theo tells him that it's everyone's place to show love. Sam reluctantly kisses Steward on the forehead, and then Theo takes Sam to his room. When they arrive, Sam says that it isn't his room and Theo says that it is and Sam should go in.
They go in and Sam is on the bed, unconscious. Sam insists that people wouldn't leave him there to rot, and Theo tells him that he was celebrated but not loved. Theo talks about how Sam surrounded himself with beautiful people who had a taste for beautiful things, but they didn't have a taste for the dying Sam's kind of beauty. When Sam wonders what happened to him, Theo explains that his body lost the ability to fight off all infections. Sam says that he'll pay for any medication, and Theo says that money can't buy him out. The Sam on the bed goes into convulsions and begs for help.
Sam wakes up in a cage with no one there. He wonders where he is, and Henry dressed as a guard steps out in the shadow and says that they're going to have a little fun. Henry picks up manacles, and says that he and Sam are mostly misunderstood. Henry insists that the only difference is that he doesn't believe his pain is greater than anyone else's. He gives a whip to another man, who goes over and whips the back of a man strapped to a prisoner. Sam insists that there's an art to the pain he gives, and begs Henry to make him stop. Henry tells Sam to look at the man, who is Sam's father, and Henry tells Sam it's where all violence begins.
Everything stops, and a man appears in a chair. A hooded man rips open his shirt, and Henry says the man worked at the finance firm where they would never accept Sam. As the hooded man attaches clamps to the financier's nipples. Sam says that he wanted self-respect and Henry asks him if torturing is what Sam calls self-respect, as the hooded man rips out the financier's nipples.
The light goes down and then comes up on Sam now strapped to a chair, and the hooded man walks around. Sam says that he's not afraid of Henry, and he's the one who should be feared. Henry tells Sam that it's time to face his fear and chaos, his shame and shadow.
Sam pulls himself free and finds himself on a beach. Big Daddy appears and walks toward him, and Sam runs. Big Daddy chases after him, and Henry appears on the beach and tells Sam that he can't outrun him. Big Daddy grabs Sam, and Henry says that the darkness always wins and all they can do is transcend. Sam's captor lets him go, and men with antenna appear on the rise. Sam turns and looks at Big Daddy, then reaches up and pulls off his hood. The man underneath is a young blonde man, who grabs Sam's head and kisses him. Sam kisses him back, and Henry tells Sam that he did well. Henry is holding an urn, and opens it and dumps the ashes inside into the water sweeping up on the beach.
1987
At the hospital, Gino brings flowers to Patrick's room. Patrick is in bed, dying, and Gino puts the flowers in water and confirms Patrick's IV drip is empty. He calls for the nurse but gets no response, and then brings water to Patrick. Patrick drinks it and Gino strokes his cheek. Gino kisses Patrick on the fore head, and tells him that he has his meds. A doctor comes in and tells Gino to leave, but Patrick insists that Gino stay. The doctor tells Patrick that virus detached both of Patrick's retinas from the back of his eyes, and he's permanently blind. AIDS patients aren't eligible for the treatment necessary, and Gino complains that the doctor can't do anything. Patrick asks Gino to just be here with him, and he's scared.
Gino gets Patrick some applesauce, and Patrick tells him to sell his life insurance policy and it will pay for his funeral. He wants the money to go to Gino, not his parents, and Gino promises. He tells Gino that he loves him.
Later, Patrick lies in his bed with a new IV. He drops his buzzer and calls for help, but no one responds. He gets out of bed but falls, and manages to put on a bathrobe and then calls for help but gets no answer. Patrick feels around and finds a wheelchair, gets into it, and wheels himself out into the hall. He hears someone ahead of him, and Barbara appears and tells him to open his eyes and look at her. Barbara appears wearing a wedding dress, in a bright light, and Patrick can see her. She removes her veil, and he asks if she's there to punish her. Barbara says that there's nothing left of him to punish, and she's there to help him see. She takes his hand, and he gets up and walks with her into the light.
Barbara takes Patrick to a crime scene, where a man in an angel costume was killed. It's the day after Halloween, 1980, and Gino is there to report on the death. Patrick and Gino talk about the man's death, and Barbara describes how Patrick was drawn to Gino. Gino gives Patrick his number and walks off, and Patrick pockets the paper.
August 1977, there's a holdup at a bodega and Patrick's partner Steve shoots the suspect. Patrick tells Steve to get cleaned up, and Barbara tells the real Patrick that Steve shouldn't have pulled his trigger, and now Patrick had blood on his hands. The 1977 Patrick tells Steve that he would have done the same thing, and then kisses Steve, who kisses him back. Mulcahey comes in and sees them, and Patrick knocks Steve away and claims that Steve came on to him. Barbara tells the real Patrick that his lies broke him into pieces.
The couple find themselves in the hospital basement, where Patrick is cutting up a corpse. Whitely is there and Patrick says that he doesn't want to do it. When he says it's ugly, Whitely doesn't care and tells him to cover up his scars and wear a uniform. When Patrick refuses to put a head on the corpse, Whitely takes a Patrick head out of a cooler and puts it on the body. Barbara tells the real Patrick that he tore himself apart and kept putting him together, and his body will keep reminding him of the pain until he faces it.
Barbara and Patrick appear in the woods while Patrick is a young man, and his father is showing him how to shoot a revolver. The young Patrick keeps missing, and his father tells him that he's "limp wristing" and should make his father and grandfather proud. When the young Patrick misses again, his father takes the gun, reloads it, and shoots with the gun next to young Patrick's ears. The boy clutches his ears as does the present-day version. When the father walks away, the young Patrick looks at the blood coming from his ears.
In 1987, Gino sits next to Patrick and holds his hand. Patrick says that he's glad he's there, and he's sorry. Gino tells him that he has nothing to be sorry about. When Patrick says that he doesn't know what's real, Gino tells him that it doesn't matter and he's got him. Patrick looks over and sees Barbara, and Kathy appears in a spotlight next to Barbara and sings. As Patrick looks off into space, Gino watches him and cries. Big Daddy appears, and Patrick flatlines. Gino lies on the bed next to Patrick and puts his cheek on Patrick's forehead. The orderlies take Patrick's body away, and Gino sits there for a while, then goes.
