Sex & Nudity

  • The term "cocksucker" is used several times
  • Harper mentions getting some "hot chick action"
  • During one of the levels, the player will enter a type of club where men and women are seen dancing. There is nothing particularly sexual about this scene, but the women do have some cleavage on display, and there is a silhouette a stripper dancing around a pole.

Violence & Gore

  • In the campaign when you shoot someone on a rare occasion it will take off one of there limbs, however if you turn on the less graphic option in the settings this will not happen.
  • In the opening scene of the first campaign mission, a guy is shown burning to death and screaming while trapped in a vehicle. Mason tries to break him out but he succumbs to the fire and dies. This scene is very hard to watch and even traumatised some viewers.
  • In a couple of missions, you can acquire a machete as a melee weapon and cut the enemies limbs off or decapitate them.
  • If someone is shot in the head, blood and brains splat on surfaces.
  • It is way more violent and intense than Black Ops (2010).
  • In the mission "Time and fate", Menendez sister Joesephina is grabbed by an enemy and thrown against a mirror. Menendez grabs a piece of shattered glass and stabs him in the neck repeatedly til' death.
  • Whenever someone is shot with a pistol, SMG and light weapons, a very large amount of blood gushes and may stain the environment. Bulletwounds will also appear and if the enemy is shot more, multiple wounds will also appear. Unlike Black Ops 1 and World at War, blood will not continue to splatter all over the surrounding.
  • With more heavy weaponry, Light/Heavy machineguns, Sniper rifles and shotguns, can on rare occasions blow their limbs of. This's very tame, but when it happens it's very graphic. In the level "Pyrrhic Victory", a man trapped in a vehicle burning alive. In the beginning of the mission we see his bloody handprint on the window. We then hear screaming and see his charred flesh and face. It's very hard to watch this scene.
  • Later in the level, the player will find a crate filled with rotting bodies. This scene is very disturbing, graphic and disgusting.
  • Zombies:
  • The Zombies mode is very gory and intense. Whenever a zombie is shot, lots of blood will appear, but generally, their limbs can easily be blown off with most weapons. Zombies heads will frequently explode when shot. Often, if a zombie is shot in the stomach region, guts fly all over the room.
  • Multiplayer:
  • This is where most gamers will spend the majority of their time. Blood still gushes from wounds and frequently stains the environment when enemies' are shot. If you get a headshot on a character, the character's head will jerk backwards sending a few chunks of gore but in actual gameplay it is not very obvious at all. In fact, you can't really notice unless you are very close or it is on a final kill cam.
  • Do note that all of the explicit blood and gore can be deactivated in the options menu, making the violence much less graphic.

Profanity

  • Definitely has the most swearing out of any Call of Duty game to date.
  • There are 93 uses of fu*k (although this varies by the story choices you make, plus there are 5 more in post credit scene, so the total number of f**k is 98), 61 uses of sh*t, 39 uses of damn, 23 uses of hell, 24 uses of bast*rd, 18 uses of bit*h, 13 uses of ass, 7 uses of coc*suc*er (all said by Admiral Briggs), 11 uses of god, 2 uses of piss, 1 use of pr*ck, and 1 use of wh*re.
  • The game actually has strong language F bombs, Shits, damns, but when you first start the game it gives you the option to a graphic content filter that mutes swearing and tones down violence and gore.

Alcohol, Drugs & Smoking

  • The main villain in the campaign is a drug dealer. You will raid one of his dens and find packages containing cocaine within. One cutscene depicts a man snorting cocaine from a rolled up dollar bill. This's the only depiction of drug use in the game.
  • There is some alcohol consumption and smoking in the game, but not very much of it.
  • Woods states that some men beating innocent civilians are "coked out of their minds".

Frightening & Intense Scenes

  • This game is not as violent as something like cod world at War but still is very dark and violent.
  • The majority of this game is not frightening, but the atmosphere is very serious, and there are many points where the violence can get very disturbing. One example is a scene where one of your comrades is trapped in an inflamed container, burning alive while desperately screaming as you try to break him out. He does not survive.
  • The game gets rather emotional at specific points. The ramifications of the player's choices and actions can affect them emotionally, such as the death of any main character.
  • The zombies game mode may be frightening to some.

Spoilers

The Parents Guide items below may give away important plot points.

Violence & Gore

  • In the mission Odysseus, should Farid dies during previous mission, and if Karma is onboard Obama, then DeFalco will slice Karma's throat, with extremely graphic and bloody results. One of the goriest moments in the game, but it can be skipped should the player performs other options.
  • In the level "Old Wounds", Kravchenko is tied to a chair for interrogation, during which Woods stabs a knife directly into his hand. Kravchenko is shown reacting in pain. It is not particularly graphic, but it is very disturbing. The same interrogation features Woods cutting a piece of flesh out of Kravchenko's face, next to his eye. This is only a little graphic but is very disturbing and intense.
  • In the level "Time and Fate", the players take on the role of the main villain going on a very violent rampage with a machete and shotgun. Ample amounts of blood and gore are present and the screen is glowing red.
  • In the level "Suffer with me", Menendez ties Hudson to a chair, picks up a shotgun, and shoots him in both kneecaps, causing much visible blood and gore. Hudson is seen reacting in agony. Menendez then cuts his throat with a necklace. This scene is very graphic and difficult to watch.
  • In one of the endings, Menendez will violently kill Woods with a knife. After this, he digs up his sister's grave. Seeing her dead body, he douses himself with gasoline and commits suicide by burning himself to death.

Frightening & Intense Scenes

  • In the campaign level "Time and Fate", the player plays as Raul Menendez as he goes on a murderous rampage with a machete. The enemies have a chance at being decapitated and the level overall is very bloody. Blood splatters everywhere anytime the player uses the machete (which is required).
  • Raul Menendez tries to save his sister from a grenade that was thrown wrong. He does not succeed. No gore is shown, but the scene may be upsetting
  • In the mission "Suffer with me", Woods is tricked into killing Mason and is shot in both knees by Menendez, which shows lots of muscle tissue and bone. Then we see another character, Hudson tied to a chair. When he reveals that he has a wife and two children, Menendez shoots him in both knees and then cuts his throat with a necklace. We then see a young boy mourning over his father's body. This entire sequence is very emotional, and graphically violent.
