- Dr. Suchong has his chest punctured by Big Daddy. Very graphic.
- Like other Bioshock games, the gameplay consists of shooting enemies with various types of guns (revolver,shotgun,machine gun etc) resulting in blood splatter and screaming. You can also melee your enemies. Using "plasmids" (fictional substances that you to shoot things out of your hand) you can set people on fire, freeze them and shatter them etc.
- Throughout the game you encounter a lot of dead bodies, some are bloodstained or have blood pooled underneath them.

- The whole game has quite a dark and bleak tone throughout and deals with dark subject matter.

Violence & Gore
- At the end of the game the main player character (Booker) is suddenly and shockingly impaled and drilled from behind by a Big Daddy's drill arm. Lots of blood splatters out and some onto a characters face while Booker screams and sputters. This is extremely unexpected and may disturb some. This is shown in first person perspective.
Frightening & Intense Scenes
- Elizabeth dies at the end of Episode 2. Very depressing and sad scene.
- At the end of the game a character has a flashback that strongly implies that a baby was decapitated by a closing portal as she was being kidnapped. This is only indicated by the portal closing around her head in slow motion followed by the screen cutting to white before the death is shown and a dolls head falls on the ground in impressionistic style and there are a few references to a child that died.
- The endings plot twists can be upsetting and disturbing and the end scene may disturb and shock some.
