I'm not sure about the meaning of the French phrase "La reine des pommes". If translated directly, it'll be named as "The Queen of Apples". But the movie has nothing to do with apples. The American name of the movie is "The Queen of Hearts". I think it indicates us that it's all about finding true love.
It seems like the "former appearance" of Pierre is some kind of "fake love". It's just a naive girl's imagination of love relationship. She believed love should last forever at first. So, after love faded, she abandoned herself in the imagination. The imagination covers up the difference of those 4 men. They look the same in her view. Gradually, her desire for love turned blind. She couldn't tell true heart from dirty flirt. Eventually, after the dramatic accident happened, she fell into a faint, and that put all these upside-down things to an end. Her fake love imagination was proved to be an illusion. She could finally see the true appearance of Pierre, who is the only one treated her with true heart. I think the director pick some typical scenes to portray the growth of an inexperienced young girl. Ingenious.
