First time I watch the program. My honest opinion on S1E7: Young lawer Nav receives Roaccutane for her acne without considering (a fairly obvious inclusion in the differential diagnosis) a condition named PCOS, also known as Polycystic Ovarian Syndrome. Her clearly visible hirsutism associated with the acne, is a giveaway sign. The acne will indeed improve with the use of Roaccutane, but the serious long term effects of the potential syndrome are in fact not considered. PCOS health complications, besides fertility problems, include risk for developing insulin resistance and prediabetes/diabetes, abnormal cholesterol- and triglyceride levels, heart disease, stroke, endometrial cancer, obesity, and sleep apnea. Maybe I am taking too much time with my patients, but I prefer to be rather cautious and thorough, trying not to miss the primary diagnosis. Instant reslults might be amazing for the here and now, but allow the primary condition to progress and lead to a potentually devestating outcome.
